Step 1
Users (creators and approvers) should be nominated by the department head of their department; and

Note : VPN connection to the PolyU network is required when it's submitted from remote location (e.g. at home or outside campus)
Step 2
Nominated users should sign an undertaking form; and
(Undertaking will be sent to nominated user via email after nomination is endorsed)
