Health and Cognitive Assessment Service

Providing comprehensive physical, functional, cognitive and mental health assessment for older adults

The Health and Cognitive Assessment Service (HeCAS) aims to provide comprehensive physical, functional, cognitive and mental health assessment for older adults. The assessments are conducted by experienced registered nurses specialized in cognitive health. Through comprehensive assessments of health and cognitive functions, we work with clients to identify their health and cognitive problems, if any, and to find ways to manage them. After the assessments, we provide information and recommendations to clients and their families, which may include an explanation of the disease process, further investigations, caregiving techniques and tips for communication.

Special Features and Advantages

Early screening allows clients and their caregivers to find out whether the clients’ cognitive complaints/observed cognitive symptoms are abnormal, and whether timely and appropriate actions, e.g. watchful waiting, diagnosis and treatment, are needed.
Recommendations given to clients and their caregivers through health education corresponding to the problems identified enable clients receive quality care throughout the course of illness.

Applications

The HeCAS is a screening and health counselling service. We provide consultations to all clients and their caregivers. We also accept referrals from social/health care professionals.
